2024 Apple watch health apps - Bought a new Apple Watch series 5 and paired it with my iPhone Xs max. In the first days I saw the monthly challenges but recently they are gone. Have never unpaired the watch. Health app is showing 2 instances of my watch, and the one that receives activity data is showed as not paired to my iphone. iOS and watch is updated to latest versions.
